WAIVER

Important: All attendees must have a completed waiver on file before participating in any LUMA GIRLS event. You can fill out the waiver ahead of time using the Google Form linked below or complete it on-site before the event begins.

 

To keep things simple, we ask parents or guardians to complete a waiver once per year. This waiver covers all events during that year.

 

If your contact or health information changes, please let us know so we can keep our records up to date. For special events, we may request an additional waiver or consent.
